Concert Health is looking for a Patient Connection Specialist. The Patient Connection Specialist will report to the Director of Patient Engagement. This role is part of the larger Patient Connection team that interacts with patients to help enroll them into Concert Health’s Collaborative Care program. This role uses rapport building, active listening, and critical thinking to qualify patients and help guide them through the enrollment process. The Patient Connection Specialist is a critical player in assisting patients in accessing high-quality care as well as meeting company goals. This person should be proactive, empathetic, and highly self-motivated. 

This is a high call volume (inbound and outbound) position.

Responsibilities 

  • Perform daily outreach to prospective patients to enroll them with Concert Health.
  • Respond to inbound calls and voicemails with the highest level of proactivity and customer service.
  • Utilize a variety of electronic medical record software for notes and documentation of patient calls.
  • Explain medical benefits thoroughly to patients during the enrollment process.
  • Answer patient questions relating to the enrollment process, billing, scheduling, etc. 
  • Participate in post-enrollment follow-up activities. 
  • 60+ inbound & outbound calls per day

Qualifications 

  • 2+ years of sales and/or customer service position, healthcare experience preferred.
  • 1+ year of call center experience preferred. 
  • Bilingual (English/Spanish) preferred.
  • Understanding of electronic medical records software, Salesforce or Zendesk
  • Demonstrate the ability to problem-solve and multi-task in a fast-paced environment.
  • Excellent verbal and written communication skills.
  • Patient focused being proactive and empathetic.
  • Collaborative Care experience a plus
  • Direct patient care experience a plus
  • Healthcare billing or revenue cycle operations a plus

Schedule 

  • Full - Time 40 hours/week fully remote position 
  • Monday - Friday 10am-6:30pm or 11am-7:30pm Eastern Time
